We are currently looking for a dedicated Social Worker to join our team and make a meaningful impact by offering emotional support, advocating for residents' needs, and guiding families through the complexities of both short-term and long-term care.
Key Responsibilities:
- Assess the social, emotional, and environmental needs of residents and their families.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide emotional support, counseling, and crisis intervention to residents and families.
- Assist new residents with the transition process and connect families with available resources.
- Advocate for residents’ rights, ensuring they are treated with dignity and respect.
- Help residents access community resources and support services as needed.
- Maintain thorough and accurate documentation of social work interventions and resident progress.
- Participate in care plan meetings and collaborate with the multidisciplinary team.
- Ensure compliance with state and federal regulations related to social work services.
Requirements:
- Bilingual in English and Spanish
- Previous experience in long-term care or healthcare settings is preferred but not required.
- Strong communication, interpersonal, and organizational skills.
- Compassion, patience, and a genuine passion for helping others.
- Ability to work independently and collaboratively as part of a team.
